Horizons Health & Wellness Center offers a variety of services including   

psychiatric evaluations, medication management, individual, couples, family and group therapy. We also offer a Psychiatric Rehabilitation (PRP), which is an additional level of support for clients that continue to struggle within their community even though their receiving medication and or therapy.  

The first step in starting services with our clinic is to complete the preregistration paperwork online. (click here) to be redirected to our secure registration application.

The second step; after the paperwork is received electronically, our staff will contact you to schedule an assessment/intake and you will be granted access to your client portal.

Horizons Health & Wellness Center accepts Maryland Medicaid and Cash payments only, at this time. CareFirst, Blue Cross Blue Shield and Medicare, will be accepted soon. Please contact us for more information.

PRP stands for Psychiatric Rehabilitation Program. It is a structured service designed to help individuals with mental illness achieve their goals, improve their quality of life, and integrate into the community. PRP focuses on recovery and empowerment, offering a range of services tailored to each individual's unique needs. Key components include assessment and planning, skill building, therapeutic activities, case management, peer support, and family involvement. These programs aim to help individuals develop the skills and confidence needed to lead fulfilling lives, manage symptoms, and achieve personal and professional goals. Horizons offers the program to individuals with similar experiences.  

The qualification process for a Psychiatric Rehabilitation Program (PRP) involves several criteria, including a documented diagnosis of a serious mental illness, evidence of significant functional impairment, referral and assessment , development of an individualized rehabilitation plan, commitment to participation, insurance coverage, and program-specific criteria. The goal is to match the individual with a program that can effectively support their recovery and rehabilitation needs. The process may also require pre-authorization from insurance providers or sliding fee scales based on income. It is crucial for individuals and their families to contact the program directly for detailed eligibility criteria and application processes.
