Horizons Health & Wellness Center is an Outpatient Mental Health Clinic that meets each client where they are, through therapeutic interventions

The Psychiatric Rehabilitation Worker is responsible for carrying out services and supports that meet the clients medically necessary mental health needs reflected in the client's individual rehabilitation plan (IRP) promoting independence, freedom of choice and overall wellness. You will render direct psychiatric rehab counseling services to clients under your caseload during the first week of each month and document contact notes for each client.  

The Psychiatric Mental Health Nurse Practitioner (CRNP-PMH) will provide outpatient psychiatric evaluation and medication management services at Horizons Health & Wellness Center Psychiatric Services department, CRNP-PMH will work in coordination with Horizon’s Health & Wellness Center, outpatient counseling services to provide psychiatric services to adult and youth clients with behavioral health issues.
