The Psychiatric Mental Health Nurse Practitioner (CRNP-PMH) will provide outpatient psychiatric evaluation and medication management services at Horizons Health & Wellness Center Psychiatric Services department, CRNP-PMH will work in coordination with Horizon’s Health & Wellness Center, outpatient counseling services to provide psychiatric services to adult and youth clients with behavioral health issues. This position will be supervised externally by a psychiatrist with whom there is a Collaborative Practice Agreement established upon hire. The CRNP-PMH will work out of Horizons Health & Wellness Center office sites in Waldorf, MD. Services will initially be telehealth services with occasional face to face until further notice. The CRNP-PMH will work in collaboration with a Medical Assistant to coordinate client care, assist with insurance pre-authorization, and caseload management.
Job Requirements and Qualifications:
Minimum Required Education:
• Master’s or Doctorate Degree in Nursing with focus on Psychiatric nursing
• Graduation from a state-certified advance practice program specializing in psychiatry/mental health
Minimum Required Experience:
• At least 1-2 years providing direct client care to persons receiving treatment for psychiatric disorders
• Ability to work with both adult and youth/child populations
Preferred Experience:
• 2+ years of experience as a psychiatric provider
• Experience in telemedicine
Licenses/Certifications:
• Must be certified by the American Nurses Credentialing Center (ANCC) in Psychiatric Mental Health (PMH) as a Psychiatric Mental Health Nurse Practitioner (CRNP-PMH)
• An unrestricted license to practice as an advanced practice registered nurse (APRN) in the state of Maryland
• Must have a valid DEA number and unrestricted DEA license in Maryland
• A state-issued Controlled Substance Registration (CSR) in Maryland
• Registration with the state Prescription Monitoring Program (PMP) in Charles County and for Maryland
(When state-wide PMP is developed), where direct services will be provided
• No suspension/exclusion/debarment from participation in federal health care programs (e.g., Medicare, Medicaid, SCHIP)
• Maintain credentials and licensure with all major insurance companies and licensing boards with the assistance of Horizons Health & Wellness Center credentialing staff. Other Skills Required:

Working Conditions / Environment:
• Full time equates to 40 working hours per week. Part Time hours TBD.
• Flexibility in setting work schedule, with work hours falling between 8:00 am and 7:00 pm.
• Continuously meet minimum required hours of direct services to clients each week, allowing remainder of work hours for documentation, meetings, trainings, and supervision.
• The noise level in the work environment is usually low. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to stand, walk, and sit. The employee is occasionally required to reach with hands and arms; climb or balance; and stoop, kneel, or crouch.
Essential Duties and Responsibilities:
• Responsible for providing psychiatric services to clients in a variety of clinical settings during scheduled hours. Scope of services includes outpatient psychiatric evaluations and medication management services.
• Assess, diagnose, and treat adults, adolescents, and children experiencing symptoms of a behavioral health disorders.
• Prescribe, recommend, or discontinue drugs and treatments to treat mental, emotional, or behavioral disorders in accordance with the Collaborative Practice Agreement and the state’s Nurse Practice Act.
• Collaborate with counselors, social workers, or other treatment team members to coordinate client care.
• Responsible for timely and accurate completion of medical records, including assessment, treatment plan, progress notes, outcome measures, and billing entries.
• Counsel clients during appointments, as necessary.
• Design/participate in the development of individualized treatment plans, incorporating best and leading practices into treatment interventions.
• Examine or order laboratory or diagnostic tests for clients to provide information on general physical condition and mental disorders.
• Collaborate with guardians, relatives, and significant others on client’s conditions and treatment when clinically appropriate.
• Prepare and submit assessments, progress notes, reports, and summaries to entities/agencies (funders, insurance companies, etc.) as required.
• Return calls and emails promptly (within 24 hours of receipt or sooner if clinically necessary)
• Respond to client emergencies and be accessible
• Responsible for making independent decisions based on protocols developed utilizing national guidelines for treatment interventions.
• Responsible for maintaining customer relations with all agency staff and clients.
• Maintain medical records and submit at least 10% of records to the supervising Psychiatrist for biweekly review in accordance with the Collaborative Practice Agreement.
• Records all services performed for accurate billing for services provided in compliance with all local, state, and federal laws, as well as payor rules and regulations. Submits billing documents in a timely fashion.
• Looks for ways to improve and promote quality. Provides teaching and education to clinical staff, if requested.
• Follow all procedures and protocols as per Horizons Health & Wellness Center policy, federal and state regulations, standard practice, and in accordance with the Collaborative Practice Agreement.
• Actively participate in all agency and department education, training, and in-services to confirm understanding of new information or changes to established clinical policies and procedures

Requirements:
Any person employed as a PRP Consultant by Horizons Health & Wellness Center PRP must:
· Have access to reliable transportation, insurance for the vehicle, maintenance of vehicle
· Valid operator’s permit that is state issued
· Have eight hours of PRP training and successful orientation completion before independently providing PRP services
· Successfully complete skills and competency-based assessments required by law or company policy
· Have minimum of high school diploma or equivalency certification
Duties:
· Responsible for carrying out services and supports that meet the clients medically necessary mental health needs reflected in the client's individual rehabilitation plan (IRP) promoting independence, freedom of choice and overall wellness
· Render direct psychiatric rehab counseling services to clients under your caseload during the first week of each month and document contact notes for each client
· Ability to work in fast paces, somewhat stressful environment
· Ability to maintain professional boundaries and provide effective supports with clients
· Ability to effectively manage timekeeping, submit required documentation as required by policy and regulations
· Ability to research community resources to best serve clients
· Ability to market services to potential clients who request additional information that best represents the mission of Horizons Health & Wellness Center PRP
· Ability to write effectively in order to submit documentation that is legible, neat and accurate
· A direct support worker is expected to facilitate psychiatric rehabilitation, behavioral support services, and crisis intervention for assigned clients, according to Individualized Rehabilitation Plan, by:
o teaching and instruction
o encouragement of positive change
o skill demonstrations and practice
o promote individual wellness, self-management, and recovery
o assist in the restoration and/or improvement of self-care social skill
o Ensure clients have an adequate supply of medications and monitor medications on a regular basis for each client
o assist with access to entitlements
o Facilitate effective strategies for strengthened independent Living and the achievement of self-sufficiency goals
o Expose individuals to academic, vocational, and employment training and resources
o Facilitate culturally relevant activities directed toward improved social relationships
o Encourage Health Promotion, Proper Nutrition, and Exercise
o Be available to clients in the event of a mental health crisis while on duty and after hours
o Ensure clients make appointments
o Must collaborate with the client's therapist, nurse case manager, mental health case manager, psychiatrist and other persons involved in the client's care to ensure the client complies with treatment schedules on a monthly basis (in some cases weekly)
o If the client resides in a community-based group living arrangement, assist the client with needs as it pertains to said arrangement in addition to other psych rehab needs
· Document incidents, hospital admissions in contact note
· Complete assessment and signed consent documentation during the first face-to-face contact.
· Complete contact note within 48 hrs. of last contact with consumer
· Complete all IRPs within 30 days of consumers admission date
· Attend individual supervision bi-weekly or as scheduled
· Attend staff meetings once per month or as scheduled
